2. Request or Print a Vote-by-Mail application

Vote-By-Mail applications can be requested online by clicking the "Request a
Vote-By-Mail application" button below, printing a blank copy from the "Print a
blank Vote-By-Mail application" link below, or over the phone by calling our office at
216-443-8683 (VOTE).
